Leveraging Technology to Support Safer Transitions of Care

SUMMARY:
Post-hospital contact with a primary care team is an established pillar of safe transitions. The prevailing model of telephone outreach is usually limited in scope and operationally burdensome. In this session, Eric Bressman, MD, MSHP, Fellow, National Clinician Scholars Program, University of Pennsylvania and Corporal Michael J. Crescenz VA Medical Center will share learnings from a study to evaluate use of an automated text message-based program to reduce hospitalizations and emergency department visits.
