Nursing Home Readmission Affinity Group Session 5: Engaging the Interdisciplinary Team

SUMMARY:
Participants will learn how a facility integrated its interdisciplinary team into patient education on the CHF Zone Tool to improve patients’ self-management of their health and wellness and reduce avoidable ED visits and hospitalizations. In addition, we will discuss how communication and collaboration among the interdisciplinary team positively impact the discharge planning process.

IN THIS SERIES, ATTENDEES WILL:

1. Identify how to integrate the interdisciplinary team into the resident/patient education and discharge process.
2. Discuss tools and resources available to assist residents/patients with embedding their disease management into their everyday life.
3. Evaluate resident/patient-centered health equity and health literacy aspects that can assist with reducing readmissions and emergency department visits.
