for Long-Term-Care Facilities

Alliant worked with a nursing home corporation in NC (12 facilities) to improve the influenza vaccination rates from last year (22/23) over a 6-month period (September 2023-February 2024). Alliant’s target goal was 10% over baseline (last year’s rate) or the State average (whichever was higher).

We used the monthly engagement model where facilities were invited to attend a virtual form to share vaccination rates, discuss challenges, and evaluate their progress. We incorporated Alliant developed resources, Seasonal Vaccination Calendar, Immunization Toolkit, and Adults Need Shots Too. We addressed motivation barriers using the CDC model “Vaccinate with Confidence: SHARE.”

Preliminary data indicates significant improvement across the majority of facilities (11/12). The average rate of improvement among the facilities was 38%. Key contributing factors were a heightened awareness of the importance of vaccinations and making vaccines universally available. Monthly meetings, individual check-ins, and leadership support emerged as pivotal elements in the success of the campaign. The initiative underscores the importance of sustained efforts in promoting flu vaccinations, ultimately saving lives, reducing hospitalizations, and enhancing the overall quality of life for Medicare beneficiaries.
