Strategies to Support Care Partners and Improve Care Transitions to Reduce Readmissions

Join the conversation as we introduce you to two leaders in caregiver support and care transitions: Jennifer Heston-Mullins, PhD, LISW will walk us through Miami University’s Our Family, Our Way, communication and care coordination resources for caregiving families to help identify gaps and conflicts, and Mark Tole, PhD, RN from UNC-Chapel Hill will share lessons learned from the Connect-Home transitions of care pilot which can lead to an improved transitional care experience.
