This interactive event highlights the importance of collaborative medication reconciliation to ensure accurate and safe medication management for nursing home residents during and after their stay. Practical examples will be used to help attendees gain a deeper understanding of how teamwork and an interdisciplinary approach can improve medication safety and quality of care.

LEARNING OBJECTIVES:
1. Understand the different roles of health care providers, pharmacists, staff, residents, and care partners in the medication reconciliation process.
2. Gain a deeper understanding of how an interdisciplinary approach can uncover “near misses” resulting from variations in the medication reconciliation process.
3. Use a performance improvement team to apply the Swiss Cheese Model to identify and address system failures in the medication reconciliation process.
